New patients are always welcome at Generations Family Practice. If you have scheduled an appointment, please arrive 30 minutes before your appointment in order to enter your information into our unique Phreesia electronic patient data entry system.

Please bring your insurance cards and a list of the medications that you are currently taking. Co-payments need to be paid at time of service, and you are responsible for co-payments or deductibles that will not be paid by your insurance policy.

Generations Family Practice offers a discount for self-paying patients who pay at the time of service. If you have any questions or concerns about the services or procedures we provide, and/or would like to schedule an appointment, please contact our office at 919-852-3999.
