New Patients

New patients are always welcome at Generations Family Practice. If you have scheduled an appointment, please arrive 30 minutes in advance to provide your information for Phreesia, our electronic patient data entry system.

To save both you and our practice time, please visit this link before the day of your appointment and provide the information requested.

Be sure to bring your insurance card and a list of the medications you are currently taking. Co-payments must be paid at time of service, and you are responsible for any co-payments or deductibles not covered by your insurance policy.

Generations Family Practice offers a discount for self-paying patients who pay at the time of service. If you have any questions or concerns about the services or procedures we provide, and/or would like to schedule an appointment, please contact our office at 919-852-3999.
