- Co-payments are due at time of service.
- If patients miss a scheduled appointment without notifying our office, they will be charged a cancellation fee.
- To save both you and our practice time, please visit this link before the day of your appointment and provide the information requested.
- Please call your pharmacy when you need prescription refills
- For most prescriptions, when the stated number of refills for your medication has been dispensed, your pharmacist will call your Generations Family Practice Provider for further refills.
- A day or so before you come for your appointment, please click here and fill out the information requested. This will save both you and the practice significant time.
